The Building Blocks of Good Coverage

Our flexible coverage means you can build a customized plan that fits your needs and your budget. These are the basic categories of coverage, however, there are additional plan designs available. Email your representative for further details.

 

The Building Blocks

Preventive/Diagnostic Services
You choose: 100% or 80% coverage includes: Annual exam, two cleanings per year, bitewing X-rays, full mouth X- rays every 3 or 5 years, annual fluoride for children.

Basic/Minor Restorative Services
You choose: 100%, 80%, or 50% coverage includes: Fillings, root canals, oral surgery, bridge & denture repair, sealants for children.

Major Restorative
50% coverage includes: crowns, dentures, bridges, periodontics.

Annual Maximum Options
$1000, $1200 or $1500 or any other amount you choose.

Orthodontic coverage
is also available for groups of 11 or more enrolled employees, with a separate lifetime maximum. Choose from $1000, $1200 or $1500 per child.
