National Provider Identifier (NPI)

As a dental care provider, you are required to use a National Provider Identifier (NPI) as your standard unique identifier. The NPI must be used for electronic transactions, including claim submissions. You should also include your state license number and tax payer identification number on your dental claims.

Please note: You must have an NPI to use our E-Office services, including BenefitCheck^SM and Claims Exchange.

If you do not have an NPI, please register at the government website: nppes.cms.hhs.gov or call the toll free number: 1-800-465-3203.

For RI dentists only:
For primary source verification, we require a copy of the document sent by the NPI Enumerator. Once you receive your NPI, please send or email a copy of your notice from Fox Systems, Inc. to:
